Description
This is a monthly membership to help seniors remain in their home and maintain and or improve their quality of life for as long as they can. For those caring for an elderly parent or loved one, it’s for you too. You can be of most help by working with them to ensure their needs are met, wishes are respected, and their dignity is kept intact. As a member you will receive the following services:
Service Includes:
-90-minute weekly home wellness check for two people
-Comprehensive physical assessment
-Checking of vital signs
-Inspection of home environment for safety
-Medication management, which includes prefilled medication planners, refill requests, and medication scheduling
-Scheduling of medical appointments and communication with healthcare providers
-Education on health-related topics
-Weekly communication with a designated family member
